    Ceftriaxone 50mg/kg IV q12h
 MIC 0.1-1 mcg/mL:
    Vancomycin IV 15mg/kg IV q6h
 MIC ≥ 2mcg/mL:

 Ceftriaxone 50mg/kg IV q12h
