Amoxicillin/clavulanate 1.2gm IV q8h

MRSA Vancomycin 15-20mg/kg IV q8-12h

If CA-MRSA suspected Trimethoprim/sulfamethoxazole Linezolid 600mg IV/PO q12h
(80/400mg) 2 tabs PO q12h
